This Cookie Notice describes how Care for Hearts Medical, LLC  and its affiliates  (“Care for Hearts,” “us,” “we,” “our”) and authorized third parties may use cookies and similar technologies (e.g., pixel tags, web beacons, clear GIFs, JavaScript, and local storage) (collectively referred to in this Notice as “Cookies”) on our Digital Properties that link to this Cookie Notice. How are cookies categorized?

Cookies are categorized in multiple ways: by their duration, by the entity that sets them, or by their purpose.
  1. By duration. Cookies are either session Cookies or persistent Cookies. Session Cookies are stored in temporary memory and are not retained after your browser is closed. Persistent Cookies are stored on your device in a folder associated with your browser and are activated again once you visit other sites that can identify that cookie. Persistent Cookies remain on your device for the duration period set within the Cookie or until they are deleted.
  2. By entity. Cookies we set when you visit our Digital Properties are called first-party Cookies. Cookies set while you visit our Digital Properties by an entity other than us (at our direction) are called third-party Cookies. Third-party Cookies typically enable certain third-party features or functionality to be provided through the Digital Properties (such as, analytics, interactive content, and tailored advertising). The entity that sets the third-party cookie can typically recognize your device when you visit our Digital Properties and again when you visit other websites that access or use the same or related Cookies set by that entity.

What cookies do we use and why?

There are four categories of Cookies that we may use in connection with our Digital Properties.

Strictly Necessary Cookies are required to enable basic website functionality and are always active. They set up functionalities for the security and efficiency of the service requested (e.g., authentication and load balancer requests). Analytics and Performance Cookies help us understand how our Digital Properties perform and how you navigate and interact throughout the Digital Properties. They help to identify which pages and sections are the most and least popular and fix Service bugs and errors by recording and analyzing site navigation, cursor movement clicks and form inputs. They can also be used to recognize you across platforms and devices to let us know whether you came to our websites after clicking a link in a search engine or from another  referral source. For example, we use Google Analytics on certain portions of our websites to help us understand how users interact with our websites. If the Analytics and Performance Cookies are inactive with the toggle icon in the left position, you are opted-out of the sharing of your personal information or sensitive personal information through Analytics Cookies. You may continue to use the our Digital Properties without these Cookies, but we will not be able to measure things like how many individual visitors have visited our Digital Properties or how they performed.

Functional and Personalization Cookies enable our Digital Properties to provide enhanced functionality and personalization (e.g., remembering choices you make such as a username, a preferred language selection, accessibility settings, or zip code or region). They may be set by us or by our vendors. The information collected by these Cookies may include data considered sensitive personal information. If the Cookies are inactive with the toggle icon in the left position, you are opted-out of the sharing of your personal information or sensitive personal information, if applicable, through Functional and Personalization Cookies. You may continue to use our Digital Properties without these Cookies, but then these Digital Properties may not work as intended.

Advertising and Marketing Cookies are used to deliver advertising and marketing that are tailored to you to deliver advertising more relevant to your interests. They may be used to build a profile of your interests, show you relevant advertisements on other sites, and manage the performance of advertisements. They may also be used to limit the number of times you see our advertisement and measure the effectiveness of our advertising campaigns. Advertising networks usually place the Cookies on our Digital Properties with our permission. The information collected by these Cookies may include data considered sensitive personal information. If the Cookies are inactive with the toggle icon in the left position, you are opted-out of the sharing of your personal information or sensitive personal information, if applicable, through Targeted Advertising Cookies. How does Care for Hearts deliver targeted advertising?

Advertisements may be delivered to you based on your browsing activity, your search activity, your response to one of our advertisements or communications, the pages you have visited, and your general geographic location. These ads may appear on our Digital Properties or on third-party websites. Our targeted advertising vendors may be members of self-regulatory associations. What are your privacy choices?

There are a few ways you can block or manage Cookies in connection with our Digital Properties.

You can manage your Cookie preferences on HelloHeart.com by interacting with the pop-up banner that appears when you visit our website. You can revisit your Cookie settings at any time by clicking Cookie Icon at the bottom left of the page.

You can reject or opt-out of the use of Cookies for targeted advertising by moving the toggle icon from right to left on the Targeted Advertising section of the Cookie Manager and Consent Tool. This will automatically opt you out of the “sale” or “sharing” of your personal information (or sensitive personal information, where applicable), which is defined under some laws as the use of such information for targeted advertising. If you choose to opt-out, you won’t necessarily see fewer ads; however, the ads you see may be less relevant to your interests.

How to manage your Cookie Preferences through your browser. You can configure your browser to block or alert you about Cookies, but if you choose to disable some or all cookies at the browser level, some portions of the Digital Properties (and other websites you access through your browser) may not work as intended. How does Care for Hearts respond to a do not track signal?
